| 1. | Provide nurse in receiving unit verbal phone report of client’s current condition. major therapies being received e.g oxygen, IV fluids, pending treatments or procedures, special nursing care needs (need for air mattress ) |
| 2. | Gather client personnel belongings. complete clothing and valuable list |
| 3. | Gather reusable equipment and supplies at bed side · Bed pan / urinals · Wash basin · Water pitcher · Kidney tray |
| 4. | Be sure all documentation including care plan is completed and current in client ‘s medical record |
| 5. | Collect medical record, cardex , medication tickets and all forms of medication |
| 6. | Assist client in transferring to wheel chair or stretcher |
| 7. | Transport client , supplies , belongings and record to receiving station |
| 8. | Announce client’s arrival at central nursing station and introduce client to nurse assuming care |
| 9. | Transport client to new room and assist in transfer to bed |
| 10. | Sending nurse makes sure agency departments are notified of clients transfer |
