TEMPERATURE/ PULSE/ RESPIRATION

1.        
Wash hands
2.        
Remove the thermometer from the 
antiseptic solution  and rinse in cold water
3.        
Wipe the thermometer from bulb towards a 
 rotating  movement , using  a clean cotton swab. Discard swab
4.        
Read the level of mercury in good light. To read  the 
level  of mercury in thermometer , hold thermometer 
with fingers of right hand, very slowly rotating back 
and forth  until the silver  mercury measurement to 
the nearest line
5.        
Shakes the thermometer if mercury level is 
above 35 degree C or 95 F. Shake it with quick
  movement of wrist. To shake thermometer, 
hold it firmly  by its stem  and move away from 
the wall, furniture and equipment

Ask the patient  to open the mouth  and place
 the thermometer  under the tongue, ask the
 patient  to hold  the thermometer in place  by
 closing the lips and not biting  on  thermometer
In  axillary method

·         Before placing the thermometer , dry the axilla
·         Place the bulb of thermometer in the axilla so
 that the bulb is in touch with two skin folds of axilla
·         Keep thermometer in position by placing  the arm over chest
·         Do not allow clothing to come in contact with bulb 
of thermometer

Have  thermometer  in place  for 2 minutes , in axillary method 
 leave thermometer in position for 5 minutes
8.        
Count the pulse  and respirations  while thermometer is in place
9.        
Place patient‘s hand over his chest with wrist extended and the palm  
downwards. place the finger tips  over pulse point
10.    
Holding watch in left hand, start to count  the pulse rate with zero then 1,2 etc.
11.    
If pulse is regular, count the number of pulsations for half minute and 
multiply it by two. If pulse  is not regular, count for one full minute
12.    
Continue palpation  of pulse  to assess  rhythm, volume, tension, and irregularity
13.    
With the  right hand  still on the pulse count  respiration  by watching  the rise  
and fall of chest , without knowledge  of patient
14.    
If the respirations are normal, count the number of respirations in the 30 second 
andmultiply by two. If respiration are abnormal , count the rate  for one full minute 
and note pattern of breathing
15.    
Remove the thermometer after two minutes (after 5 minutes in case of axillary method). 
Wipe the thermometer from stem to bulb with a clean cotton swab, using a rotating 
movement. Discard the swab
16.    
Read level of mercury
17.    
Return  thermometer  to bottle no :2 , shaking it down again
18.    
Wash hands
19.    
Record  the temperature, pulse and respiration.
