HOSPITAL ADMISSION
1. | Wash hands and prepare room equipment and furniture. Prepare bed by adjusting it to lowest horizontal position. Turn down the sheet and spread. Arrange room furniture for easy access to bed |
2. | Assemble any special equipment such as suction or oxygen supplier. Be sure equipment is in working order |
3. | Role of admitting clerk Maintain professional relationship with client · Welcome and escort client and family to interviewing area |
4. | Provide for clients safety · Acquire identifying information including clients full legal name , birth date and address , nearest relative , physician , religion , previous admissions , occupation and type of insurance · Apply identification band to clients wrist: be sure band contains following information: clients full legal name, hospital or agency number, physician, birth date. be sure band is secure |
5. | Provide for client legal rights ‘ · Instruct client or legal guardian to read general consent form for treatment. Assess clients understanding of consent form. requesting client or family member to sign form if there is agreement to be admitted for treatment · Provide client or guardian with information about the right to make decision about medical care , including the right to accept or refuse medical or surgical treatment · Provide any brochures describing purpose of agency , organization and policies or rules that affect persons conduct as client |
6. | Role of nurse Assess client need Assign client room on basis of client condition health care needs and personal preference |
7. | Send client to screening area for necessary diagnostic testing |
8. | · Provide for continuity of care when client is admitted through ER · Notify nursing division of client admission : name , assigned room and bed , admitting physician , diagnosis , pertinent information related to client condition · Transport client and family members to nursing division using escort · In nursing division , introduce client and family to nurse assuming client care · Share pertinent observation about client behavior with nursing staff |
