Wash hands

Attach the tubing to the enema-can and clamp the tube

Prepare the solution at the required temperature. Add 30 ml of soap jelly to 600 ml of water. Test the temperature of the solution at the inner aspect of the wrist remove the froth from the can.

Hang the can with the solution on the stand and adjust the height at 45 cm from the anus
  1.  
Attach a rectal tube tubing. Loosen the screw clamp and let a small amount of fluid to run into the kidney tray. Regulate the flow of the solution by adjusting the screw clamp. Pinch the tubing with fingers. Read the level of the fluid in the enema can.

Put on clean disposable gloves.
  1.  
Lubricate the rectal tube tip to 5 cm above tip with water soluble lubricant.

Separate the patient’s buttocks to clearly visualize anus. Hold rectal tip against anus pointing in direction of umbilicus. After anal sphincter relaxes, gently insert tip 5 cm to 10 cm.

Hold the enema tube in place throughout the procedure.

During the administration of the fluid ask the patient to take panting breaths.

Temporarily stop the administration of the solution. (But do not remove the tube) if the patient has an urge to defecate or has abdominal cramps or if the solution level is rising in the
  1.  
Continue the administration to give about 500 to 750 ml of solution. stop the procedure if the patient develops discomfort
  1.  
Clamp the tubing, gently remove the rectal tube by pulling through 3-4 layers of rag pieces. Hold the rag pieces around the lube and firmly against the anus.

Discard the rag pieces in the paper bag. Detach the rectal tube and place it in the kidney tray.

Ask the patient to retain fluid while lying quietly in bed for 15 min.
  1.  
Assist patient on to toilet, commode or bed pan.
  1.  
Give call bell and leave him alone if safe.
  1.  
Explain to patient that during the next hour or so he may need to evacuate bowels several more times.
  1.  
Observe enema results, noting colour, consistency and amount. Obtain specimen to test if ordered.
