CATHERIZATION

1.	Identify clients at risk  for developing  infection  of genitalia , rinary or reproductive tract (eg indwelling catheter , fecal incontinence or surgical incision (eg episiotomy )
2.	Explain procedure  and its purpose to client
3.	Prepare necessary supplies  and equipment
4.	Assemble supplies at bed side
5.	Pull curtain around bed or close the door of the room
6.	Raise bed to a comfortable working position
7.	Lower side rail and assist clients in assuming a dorsal recumbent position
8.	Position  water proof pad under buttocks or place bed pan  under client
9.	Fold top bed linen towards foot end of bed and raise clients Gown above genital area
10.	Drape client by placing blanket with one corner between legs .one corner  pointing toward each side of bed and one corner over chest . side corners around legs and under hips
11.	Help client  flex knees  and spread legs  back to abdomen
12.	Fold lower corner of bat blanket between legs back on to abdomen
13.	Fill wash basin  with warm water
14.	Place wash basin  and toilet  tissue  over bed table .place wash clothes in basin
15.	·         Provide female genital care Wash hands and wear disposable gloves Wash and dry upper thighs Wash labia majora.then use non dominant hand  to gently  retract labia from thigh .with dominant hand, wash carefully in skin folds .wipe in direction from perineum to rectum , repeat on opposite side using separate section of wash cloth .rinse , dry area carefully ·         Separate labia with non dominant hand to expose urthral meatus and vaginal orifice.with dominant hand , wash downward from pubic area towards rectum in one smooth stroke , use separate section of cloth for each stroke , cleanse thorough around labia minora , clitoris and vaginal orifice ·         If client is on bed pan , pour warm water over perineal area ·         Dry perineal area thoroughly ·         Fold lower corner of bath blanket between legas and over perineum  . ask client to lower legs and assume side lying position for anal care
16.	Clean anal area Clean anal area first by wiping fecal material with toilet tissue .wash by wiping from vagina toward anus with one stroke . discard wash cloth . repeat  with clean cloth until skin is clear of fecal matter Rinse area well and dry with bath towel Remove disposable gloves and dispose in proper  receptacle Assist client in assuming  comfortable position Remove bath blanket and dispose of all soiled bed linen . return unused equipment  to storage area Raise  bedside rail  and lower bed to proper  height  . return room to the condition before procedure Wash hands
17.	Inspect surface  of external  genitalia  and surrounding skin  after cleaning
18.	Record  procedure  and abnormal finding (eg any discharge or condition of genitalia )
