1 | Identify the patient who requires wound dressing and the type of dressing to be performed. |
2 | Explain to patient and family about the importance of procedure and how they have to cooperate. |
3 | Spread the paper, place bag in a suitable area and take out soap and brush and other required articles from outer pocket. Loose the inner pocket cover. |
4 | Wash hands and dry. |
5 | Open inner pocket and assemble all the required articles at the bed side, table area or chair. |
6 | The work area may be arranged while the instruments are boiling. |
7 | Remove the bandage outer dressing with fingers or forceps and discard it into paper bag. |
8 | Wash hands thoroughly with soap, water and nail brush. |
9 | Create a sterile field by keeping sterile articles in the bowl/basin used for boiling or plate with which is sterilized. |
10 | Pour the cleansing solution using a artery forceps or hydrogen peroxide. |
11 | Clean the wound with cotton swab dipped in cleansing solution using artery forceps. |
12 | Apply medication as directed in the standing order with a cotton applicator. |
13 | Apply sterile dressing and fasten with the bandage. |
14 | Explain the condition of wound to the patient and family relatives and also provides instructions regarding care, rest, exercise and diet. |
| AFTER CARE:- |
13 | Record procedure, condition of wound, and medication used. |
14 | Wrap soiled dressing and burn. |
15 | Wash used instruments and bowl with soap and water and rinse and boil for 20 minutes. |
16 | Wash hands and return clean soiled instruments to the outer pocket of bag in polythene. |
17 | Inform about your next visit and refer the client to the nearest centre. |
