1. | Introducing self |
2. | Identify patient by identification band |
3. | Explain procedure to patient |
4. | Wash hands and wear clean gloves |
5. | Position the patient and bring affected area to nurses working level |
6. | Open and prepare supply in order of use |
7. | Remove cap from medication and place inner surface up |
8. | Apply medication with sterile tongue depressor /applicator or clean gloves |
9. | If area treated is extensive use a new sterile tongue depressor |
10. | Begin in center of affected area and apply medications in circular motion towards outer edge of affected area |
11. | To cover the area with dressing if ordered |
12. | To apply patch type of medication · Obtain appropriate patch of proper measurement · Place ointment inside the patch directly · Reposition patient for comfort and safety · Remove gloves and wash hands |
13. | Record in nurses record |
14. | Record skin condition before and type of topical medication administered and strength of medication and site |
15. | Report any change or abnormalities |
